UnitedHealthcare Choice Plus Plan

The UnitedHealthcare Plan provides coverage through in-network doctors and you do not need a referral or coordination from a primary care physician (PCP) when you need care. This plan also provides out-of-network doctor and hospital coverage. Trying to utilize in-network doctors and care will usually result in lower prices for you as a consumer.

You will not have access to out-of-network physicians, with the exception of emergency situations, if you are enrolled in this option. If you decide to seek treatment from an out-of-network doctor, you will be responsible for the full cost of treatment.
